List of Documents Required

  • One cheque issued by MHDI
  • Your PAN card attested by you
  • Your latest bank statement copy self attested
  • Your declaration enclosed in the format enclosed where the old instrument is being returned
  • In case of the cheque issued by the company is lost indemnity bond as per attached format to be printed on non judicial stamp paper of Rs 10
  • One cancelled cheque by your company
